Abstract

La tesis titulada "Cambios en el espectro de la meningitis bacteriana espontánea en el paciente adulto en un hospital de tercer nivel" está formada por 3 artículos y tiene como objetivo global analizar los cambios observados en las meningitis bacterianas del adulto en las últimas décadas en un hospital de tercer nivel de Barcelona. En el primer artículo, se evalúan 635 meningitis bacterianas desde 1982 al 2010, y se analizan en 2 períodos: 1982-1995 y 1996-2010. En el segundo periodo la población es de mayor edad, con mayor comorbilidad y con menor frecuencia presenta la tríada clásica. Neisseria meningitidis presenta un claro descenso, siendo la causa más frecuente de meningitis bacteriana Streptococcus pneumoniae, seguida de N.meningitidis, Listeria monocytogenes y los bacilos gram-negativos; y aumenta la realización de pruebas de imagen (TC) y el uso de dexametasona. A pesar de los cambios en la etiología y el manejo precoz y adecuado de las meningitis, a lo largo de estos 29 años no hemos obtenido diferencias significativas en cuanto a las complicaciones neurológicas, la mortalidad y las secuelas posteriores al episodio. La mayoría de las revisiones sobre las meningitis bacterianas por bacilos gram-negativos (BGN) son retrospectivas e incluyen tanto las meningitis espontáneas como las postquirúrgicas/postraumáticas. El objetivo del segundo artículo es describir las meningitis espontáneas por BGN y compararlas con el resto de meningitis bacterianas espontáneas. Para ello hemos analizado 544 meningitis, desde 1982 al 2006, de las cuales 40 (7%) fueron por BGN. Los pacientes con meningitis por bacilos gram-negativos son mayores, tienen mayor comorbilidad, presentan con menor frecuencia la tríada clásica y presentan mayor tasa de complicaciones neurológicas, sistémicas y mortalidad (20 veces mayor respecto a la meningitis meningocócica). Escherichia coli es el microorganismo más frecuentemente aislado, seguido de Pseudomonas spp. Dado que los pacientes ancianos son un segmento de la población con mayor crecimiento en la sociedad occidental y que la etiología de la meningitis bacteriana está claramente influenciada por la edad del paciente, el objetivo del tercer estudio es determinar el espectro de la meningitis bacteriana de los ancianos a lo largo de los últimos 30 años, comparándola con la población joven en la misma zona geográfica. Los pacientes ancianos tienen mayor comorbilidad, manifestaciones clínicas más sutiles, un espectro etiológico diferente (ganando protagonismo L.monocytogenes y los bacilos gram-negativos) y mayor número de complicaciones neurológicas, extraneurológicas y mortalidad global.


The thesis entitled "Cambios en el espectro de la meningitis bacteriana espontánea en el paciente adulto en un hospital de tercer nivel" is formed by 3 publications and its global aim is to determine the changes observed in the spectrum of the bacterial meningitis in adults recruited from a large tertiary university hospital in Barcelona over the last years. In the first publication we evaluated 635 episodes of bacterial meningitis from 1982 to 2010. The observation was divided in two periods: 1982-1995 and 1996-2010. Patients in period two were older, and more frequently had co-morbid conditions. They presented less frequently with the meningeal triad and a decline in meningococcal meningitis was observed, being Streptococcus pneumoniae the most frequent etiologic agent followed by N.meningitidis, Listeria monocytogenes and gram-negative bacilli. Dexamethasone therapy was significantly more used and cerebral computed tomography was more frequently performed in period two. Despite all the aforementioned changes, the rate of neurologic complications, post-meningitic sequelae and the overall case-fatality did not improve between periods. Much of the current understanding of gram-negative bacillary meningitis (GNBM) in adults is based on studies that are limited by retrospective design and analysis that combine patients with spontaneous meningitis and meningitis secondary to trauma or neurosurgery. Our objective in the second paper is to describe and compare the characteristics and the outcome of adult patients with spontaneous gram-negative meningitis and spontaneous meningitis due to other pathogens. Between 1982 and 2006, GNBM was diagnosed in 40 (7%) of 544 episodes of spontaneous acute meningitis. Patients with spontaneous gram-negative meningitis were more likely than other patients to be older, to have underlying diseases, the classic meningeal triad was less frequent and they had higher overall rates of neurological complications, systemic complications and death. The most common pathogens were Escherichia coli and Pseudomonas spp. Because elderly people are one of the fastest-growing segments in most western societies and the incidence of meningeal pathogen infections is highly influenced by the age of the patient, the aim of our third study was to determine the spectrum of the acute bacterial meningitis in elderly patients over the last 30 years, compared to a younger population from the same geographical area. Elderly patients had comorbid conditions more frequently, subtler clinical manifestations, different etiologic spectrum (being more frequent L.monocytogenes and gram-negative bacillary), and had a higher number of neurologic, extra-neurologic complications and the overall mortality ratio.
