Abstract

L’arteriosclerosi és la principal causa de mortalitat avui en dia a nivell mundial. Dintre de les seves manifestacions clíniques, aquesta tesi s’ha centrat en l’estudi dels ictus per arteriosclerosi carotídia (AC) , una causa molt freqüent d’ictus a la nostra població. Una de les principals característiques dels ictus per AC és el seu elevat risc de recurrència que disminueix de forma notable amb el tractament de revascularització si es du a terme dintre de les dues primeres setmanes. Malgrat això, un tractament molt precoç també comporta un risc d’ictus i mort periprocediment elevat i no està clar quin és el moment més òptim per a la revascularització. Als darrers anys, els tractaments mèdics de prevenció secundària han millorat significativament, aconsenguint una reducció del risc de recurrència en alguns assajos clínics. Per tant, el primer objectiu d’aquesta tesi ha estat descriure la taxa de recurrència en pacients amb AC simptomàtica (estenosi 50-99%), mentre esperen el tractament de revascularització. Hem trobat que, malgrat la millora dels tractaments, el risc de recurrència continua essent molt alt (d’un 6,6% i un 11,5% als 3 i 14 dies respectivament). L’edat i el tipus de símptoma inicial (cerebral vs. ocular) són els principals factors associats a la recurrència. En segon lloc, hem volgut analitzar quina és la severitat i el risc de deteriorament neurològic precoç (DNP) d’aquest subtipus d’ictus. Així, hem trobat que el 75% d’aquests ictus tenen una severitat inicial lleu (NIHSS ≤7). No obstant això, un 23% presenten un DNP durant les primeres 72 hores d’evolució. El grau d’estenosi és el principal factor associat a una major severitat i un major risc de recurrència. Altres factors relacionats són l’antecedent d’accident isquèmic transitori durant els tres mesos previs, que s’associa a una menor severitat però a un major risc de DNP, i la tensió arterial mitja a l’arribada del pacient a urgències, que s’associa també a un major risc de DNP. Finalment, el tercer objectiu d’aquesta tesi ha estat estudiar l’existència d’un biomarcador de risc de recurrència i DNP en aquest subtipus d’ictus. Per a això hem analitzat una sèrie de biomarcadors relacionats amb la fisiopatologia de l’arteriosclerosi com són les cèl·lules endotelials progenitores (CEP), les cèl·lules endotelials circulants (CEC), l’LDL-oxidada (LDL-ox) i la microalbuminúria (MA). Hem objectivat que un recompte baix de CEP dintre de les primeres 24 hores s’associa a un risc més elevat de nous esdeveniments vasculars dintre dels primers sis mesos, mentre que els valors d’LDL-ox i MA s’associen a un risc més elevat de DNP. Amb els resultats d’aquesta tesi podem concloure que els ictus deguts a AC continuen tenint un risc de recurrència abans de la cirurgia molt alt. Per intentar reduir-lo, semblaria recomanable iniciar un tractament mèdic agressiu i ingressar els pacients durant les primeres 72 hores. La utilització de biomarcadors pot ajudar a la selecció d’aquells pacients amb un risc més alt, que es podrien beneficiar de noves teràpies o procediments de revascularització urgents, si fos possible, dintre d’un assaig clínic.


Atherosclerosis is the leading cause of death worldwide. This thesis focuses on one of its clinical manifestations, the strokes due to carotid atherosclerosis (CA). This is one of the most frequent sources of stroke in our population and carries a high risk of recurrence. However, this risk decreases dramatically if a revascularization procedure (either surgical or endovascular) is performed within the first two weeks. Nonetheless, a very early procedure also confers an increased risk of periprocedural stroke or death. Hence, the optimal time for revascularization remains unknown. In the last decades, medical treatment has improved noticeably, and a reduction in the recurrence risk has been found in some clinical trials Therefore the first aim of this thesis was to describe the rate of recurrence in patients with symptomatic CA (stenosis 50-99%) awaiting revascularization. We found a high recurrence risk (6.6% and 11.5% at 3 and 14 days respectively) in spite of better treatment. Age and initial symptoms (cerebral vs. ocular) were the main factors associated with recurrence. The second objective of the thesis was to analyze the initial severity and the risk of early neurological deterioration (END) of this stroke subtype. Moreover we wanted to investigate the role of the stenosis degree and thus we included patients presenting with total carotid occlusion. We observed that 75% of the strokes had a low initial severity (NIHSS ≤7). However 23% suffered END within the first 72h. Stenosis degree was the main factor associated with a higher initial severity and END risk. Those patients presenting total carotid occlusions carried the highest risk of recurrence (in our cohort). Other related factors were previous transient ischemic attacks, which were associated with a lower severity and a higher risk of END, and initial mean arterial pressure that was associated with an increased risk of END. Finally, the third objective was to study the existence of a biomarker of recurrence for this stroke subtype. For this reason we studied the role of endothelial progenitor cells (EPC), circulating endothelial cells (CEC), plasmatic oxidized-LDL (Ox-LDL), and albuminuria. We found that a low count of basal EPC was associated with a higher risk of new vascular events (i.e. new stroke, myocardial infarction, vascular death or hospitalization due to a vascular disease) within the first 6 months of follow-up. In addition, we found that Ox-LDL and albuminuria were associated with a higher risk of END. Finally, we described that CEC levels were not associated with increased risk of recurrence. Collectively the results presented as part of this thesis conclude that strokes due to CA, and awaiting revascularization, still bear a high risk of recurrence. Early intensive medical treatment and admission in Stroke Units during the first 72h seems advisable. The use of biomarkers could help to select those patients at higher risk of recurrence in order to test new therapies or urgent revascularization procedures, ideally as part of a clinical trial.
