Abstract

En aquesta tesi es fa una aproximació teòrica a la salut mental i alguns conceptes associats a ella. Els determinants socials de la salut (mental) en els darrers anys han evidenciat que els humans som vulnerables a la inseguretat econòmica, a la precarietat vital i a les desigualtats socials de les nostres societats. Una vulnerabilitat que s’ha manifestat de moltes i diverses maneres en una pitjor salut mental: augment de l’ansietat i de la depressió, de consum de fàrmacs i dels suïcidis. S’aborden les desigualtats socials i com aquestes impacten en la salut mental i especialment amb les desigualtats de rendes. Unes desigualtats que han posat de manifest l’extrema importància que tenen les polítiques de protecció social i econòmica, fonamentals per a donar estabilitat i seguretat a la vida i a la salut de les persones. Es fa un repàs de les orientacions polítiques de diverses institucions i agències en salut per tal de promoure la salut mental i reduir les desigualtats socials. S’avaluen també negativament algunes de les polítiques que es porten a terme per part dels governs espanyol i català en un context de forta crisi i augment de les problemàtiques de salut mental derivades de la pobresa, l’atur i la precarietat laboral. Per fer front a aquesta situació de greu recessió econòmica i de gran augment de la pobresa de les poblacions, amb tot el que significa de patiment psíquic i problemes de salut mental, els govern espanyol i català han fet ús de les mateixes receptes que en una època qualsevol de creixement econòmic i aquestes velles fórmules en aquesta llarga i duradora crisi no han funcionat. No només han fracassat per les retallades dels diversos programes socials i serveis públics que existeixen, que han estat prou considerables, sinó per la enorme magnitud del problema que tenen les nostres societats: un atur crònic i persistent en un context on el mercat de treball està perdent llocs de treball per una automatització accelerada i una pobresa instal·lada permanentment des de fa anys que amb les temporals prestacions d’atur i ínfimes rendes mínimes no ha pogut fer desaparèixer. Tot això ha contribuït a elevar els nivells d’inseguretat econòmica que diverses investigacions han evidenciat que té greus conseqüències per a la salut mental de les persones. Finalment es planteja una mesura de protecció social de seguretat econòmica com la Renda Bàsica que té algunes experiències en diversos països. Els diversos experiments que s’han fet (Índia, Namíbia, Canadà, Carolina del Nord, Kenya, Alaska, etc.) assignant ingressos incondicionals a la població han aportat diversa evidència empírica de millores en diversos terrenys de la salut i especialment en la salut mental, la reducció de les desigualtats socials i la pobresa, millores en l’educació, en les relacions humanes, l’economia, etc. Una política de protecció social universal com la Renda Bàsica universal que s’estudia en aquest treball i que doni cobertura a tota persona pel fet de ser ciutadà, treballi o no, sigui jove o vell, home o dona, i que garanteixi una seguretat econòmica posaria fre a aquesta greu situació de precarietat vital i patiment mental d’una part cada cop més important de la societat.


The aim of this thesis is to provide a theoretical approximation to mental health and several related concepts. The social determinants of (mental) health have shown in recent years that human beings are susceptible to economic uncertainty, precariousness of their living conditions and social inequality. This vulnerability has frequently appeared as a worsening of mental health in many different ways, for example increased anxiety, depression, greater consumption of prescription drugs and suicide. This study considers social and especially income inequalities, and how they affect mental health, drawing attention to the extraordinary importance of policies aiming at social and economic protection, which are seen as essential for offering stability and security in people’s lives and health. The political orientations of a range of health-oriented institutions and agencies working to promote mental health and to reduce social inequality are considered, while critical evaluation is made of some policies being implemented by the Spanish and Catalan governments at a time of serious economic crisis and a concomitant rise of mental health problems deriving from poverty, unemployment and job insecurity. In this situation of severe economic recession and drastically increased poverty, and with everything it entails in terms of psychological suffering and mental health problems, the Spanish and Catalan governments have resorted to the same measures they have used in periods of economic growth, obsolete strategies which have proven ineffective in the long, unabating crisis. They have failed not only because of considerable cuts in already-existing social programmes and public services but also because of the enormity of the problem: chronic, enduring unemployment in a labour market which is losing jobs owing to fast-increasing automation, and deep-rooted, longstanding poverty which has resisted attempts to abolish it by means of temporary unemployment benefits and low-level minimum income schemes. All of this has contributed towards worsening economic insecurity which, as a range of research projects have demonstrated, has serious consequences for mental health. The study concludes with a discussion of Basic Income, a social protection measure offering economic security which has been tested in several countries. Experiments whereby unconditional cash payments made over different periods to target populations in India, Namibia, North Carolina, Kenya, Alaska, for example, have provided empirical evidence of improvement in different aspects of health and mental health in particular, together with a reduction of social inequalities and poverty, advances in education, human relations, and in the economic sphere, inter alia. A policy of generalised social protection like the universal Basic Income examined in this study would benefit every citizen or registered resident in the society, irrespective of whether the person works, is old or young, male or female, and this guaranteed economic security could help to put an end to the present serious situation of precarious living conditions and the mental suffering of ever-increasing numbers of people.
