Caracterització de les propietats que fan de lactobacillus el regulador de l'ecosistema vaginal i el protector contra els patògens genito-urinaris

Abstract

A la vagina humana, la microflora i el seu ecosistema han d'estar en equilibri, per a protegir-la de les colonitzacions i infeccions. També és necessària una relació entre l'hoste i la seva microflora. En l'ecosistema vaginal, el microorganisme més important pel seu nombre i per la seva activitat és Lactobacillus. <br/>La infecció del tracte urinari i la vulvovaginitis candidiàsica són dues patologies molt prevalents en la població femenina, en les quals de Lactobacillus ha demostrat tenir un paper important: evitar la colonització i la posterior infecció.<br/>Els mecanismes bàsics pels quals Lactobacillus exerceix un paper regulador en la microflora vaginal, són dos: bloqueig de l'adherència dels patògens a l'epiteli vaginal i la inhibició del seu creixement. Per a utilitzar soques de Lactobacillus com a agents probiòtics, és necesària la correcta caracterització de les propietats abans esmentades. <br/>L'objectiu general d'aquest treball és seleccionar les soques de Lactobacillus més ben dotades per ser utilitzades com agents bioterapèutics.Els objectius concrets són:<br/>1.- Averiguar quines soques són les més adherents a les cèl.lules de l'epiteli vaginal (CEV). 2.- Determinar quines són les més bloquejadores de l'adherència de microorganismes a les CEV. 3.- Comprovar si la capacitat de bloqueig de l'adherència es correlaciona amb altres capacitats. 4.- Averiguar quines soques són les més inhibidores del creixement d'altres microorganismes. 5.- Comprovar si els factors inhibidors del creixement són excretats. 6.- Comprovar si és factible concentrar els factors inhibidors del creixement. 7.- Caracteritzar els factors inhibidors del creixement. 8.- Comprovar si la capacitat d'inhibició del creixement es correlaciona amb altres capacitats.<br/>Es van escollir 15 soques de Lactobacillus d'una col.lecció previament caracteritzada pel nostre equip, i 25 soques de patògens genito-urinaris. Es van realitzar assaigs d'adherència, bloqueig de l'adherència dels patògens a l'epiteli vaginal i inhibició del creixement dels patògens, tant en medi sòlid com en líquid. De les soques més capacitades es va caracteritzar l'activitat inhibidora del creixement, mitjantçant assaigs de difusió en agar des de pous , tractaments físics i químics, SDS-PAGE i cromatografía.<br/>Dels resultats obtinguts, es conlcou:<br/>Els Lactobacillus més adherents a CEV són Lactobacillus LB35, seguit de LB25 i LB56. Per Grups d'Hemaglutinació és el Grup III, seguit del II i I.<br/>Els Lactobacillus més bloquejadors per exclusió són Lactobacillus LB35, seguit de LB30 i LB39. Per Grups és el Grup III, seguit de I i II. Hi ha diferències en la capacitat de bloqueig de diverses soques de Lactobacillus contra un determinat patogen i entre una mateixa soca de Lactobacillus contra diversos patògens. LB35 ha mostrat ser capaç de bloquejar per competició i desplaçament l'adherència de E. coli EC26 a l'epiteli vaginal.<br/>Els Lactobacillus més inhibidors del creixement en medi sòlid són Lactobacillus LB35, LB11 i LB65. En medi líquid són LB55, LB5 i LB21. El més inhibidor d'E. coli en sòlid és LB35 i en líquid LB38, essent LB35 la quarta millor soca.Els factors inhibidors del creixement són excretats per Lactobacillus, ja que els sobrenadants filtrats mantenen l'activitat, i aquesta es conserva quan es concentren per ultrafiltració, liofilització i cromatografia. La substància inhibidora excretada per Lactobacillus s'ha caracteritzat com termoestable, baroestable, crioestable, cronoestable i hidrofílica. El seu pes mol.lecular estimat per ultrafiltració és < a 5kDa. L'activitat de la substància inhibidora depen de la soca de Lactobacillus. Per SDS-PAGE l'estructura química de la substància inhibidora sembla que podria ser una proteïna, mentre que per assaigs de difusió en agar es detecta resistència a les proteïnasses. Aquests resultats suggereixen que es podria tractar d'un pèptid petit.<br/>La capacitat d'adherència de certs Lactobacillus a les CEV es correlaciona amb la capacitat de bloqueig de l'adherència del patògens i amb la capacitat inhibidora del creixement.<br/>S'han seleccionat els Lactobacillus LB35, LB39, LB59, LB65 i LB68 com a candidats a producte bioterapèutic, ja que són els que millor compleixen els requisits exigits.Aquests Lactobacillus sols o combinats entre sí, poden tenir un paper important en la regulació i l'equilibri de l'ecosistema vaginal i contrarrestar als patògens uro-genitals que hi arribin. Les indicacions preventives i/o terapèutiques més immediates podrien ser les infeccions del tracte urinari i les vulvovaginitis candidiàsiques.


In the human vagina, the microflora and its ecosystem should be in balance, in order to protect the vagina from colonizations and infections. In this ecosystem, Lactobacillus is the most important microorganism due to their number and activity. <br/>Urinary tract infections and vulvovaginal candidiasis are prevalent pathologies in women. In these situations, Lactobacillus has demonstrated to play an important role avoiding vaginal colonization and posterior infection. <br/>The basic mechanisms, for which Lactobacillus exercises a regulatory role in the vaginal microflora, are two: blockage of the adherence of the pathogens to vaginal epithelium and inhibition of the growth of this invasor pathogen. In order to use Lactobacillus strains as probiotic agents, it is mandatory the correct characterization of the previous properties.<br/>The ultimate goal of this study is to select some Lactobacillus strains to be used as biotherapeutic agents. <br/>The immediate objectives are: 1- Determine the strains with more adherence power to the vaginal epithelial cells (VEC). 2- Determine the strains with highest capacity to block the adherence of the pathogens to VEC. 3- Correlate the blockage of adherence with other capacities. 4- Determine the strains with more inhibitory activity against the growth of pathogens. 5- Check if the inhibition factors are excreted by Lactobacillus. 6- Check if the inhibition growth factors can be concentrated. 7- Characterize the inhibition factors. 8- Correlate the inhibition power with other capacities. <br/>Fifteen Lactobacillus strains from a collection previously studied by our team were selected, as well as 25 urogenital pathogens strains. Assays of adherence to VEC, assays of blocking the adherence of the uropathogen to VEC, and assays of inhibition the growth of pathogens in solid or liquid medium, were carried out. Further, the growth inhibition activity was characterized, among the Lactobacillus strains with better capacities, using assays of agar well diffusion, physical and chemical treatments, SDS-PAGE and chromatography. <br/>Conclusions: Lactobacillus LB35 is the most adherent Lactobacillus to VEC, followed by LB25 and LB56. By Groups of hemagglutination, Groups III is the most adherent, followed by II and I. In assays of blockage by exclusion, Lactobacillus LB35 is the most capable to block adherence, followed by LB30 and LB39. Within the Groups, Group III showed the highest blocking activity, followed by I and II. Differences in the blocking capacity have been detected among different Lactobacillus strains against one pathogen and in one Lactobacillus strain to different uropathogens. LB35 is able to block the adherence of E.coli EC26 to VEC, by competition and by displacement. <br/>In solid assays, Lactobacillus LB35 shows the highest inhibitory activity against uropathogens, followed by LB11 and LB65. In liquid assays, the most active strain is Lactobacillus LB55, followed by LB5 and LB21. Lactobacillus LB35 and LB38 have the best inhibitory activity in solid assays against E. coli, whereas in liquid assays LB35 behave as the fourth best strain. The inhibitors growth factors are excreted by Lactobacillus, since the Lactobacillus-free filtered supernatant maintains the inhibitory activity. Moreover, this activity is conserved in the filtered supernatants concentrated by ultrafiltration, lyophillization and chromatography. This inhibitory substance has been characterized as termoestable, baroestable, cryoestable, cronoestable and hydrophilic. Its molecular weight estimated by ultrafiltration is <5kDa. The potential of the inhibitory substance depends on the Lactobacillus strain. In SDS-PAGE assays, the inhibitory factor seems to be a protein, although in assays by agar well diffusion, resistance to proteolytic enzymes is detected. These results suggest that this factor could be a small peptide. In certain Lactobacillus, the capacity of adherence to VEC correlates with the capacity of the blockage of adherence and with the capacity of inhibits the growth of pathogens. Lactobacillus LB35, LB39, LB59, LB65 and LB68 can be selected as candidates to biotherapeutic products, since they fulfilled the demanded requirements. These Lactobacillus alone or in combination, could play an important role in the regulation and balance of the vaginal ecosystem. The immediate prophylactic and/or therapeutic indications could be the urinary tract infections and vulvovaginal candidiasis.
