Evaluación del uso de antimicrobianos como factor de riesgo relacionado con la aparición de resistencia a cefalosporinas en Escherichia coli y Salmonella en cerdos

Abstract

El objetivo principal de esta tesis fue evaluar si existe asociación entre el consumo de antimicrobianos en granjas convencionales de porcino, y la presencia de enterobacterias resistentes a cefalosporinas (RC), como Escherichia coli y Salmonella spp. A la vez, se evaluó si la granja era un reservorio de genes de RC, si éstos estaban asociados a plásmidos transferibles y si dichos plásmidos eran similares a los que están presentes en cepas humanas. Para ello se realizaron tres estudios. En el Estudio 1 se evalúo si los tratamientos con ceftiofur y posteriormente con amoxicilina eran factores de riesgo en la emergencia de E. coli RC en una granja de cerdos de engorde. Para ello se seleccionaron 100 lechones de 7 días de edad y se dividieron en dos grupos, un grupo control (n=50) y otro grupo tratado parenteralmente con ceftiofur en dosis única (n=50). Durante el período de engorde, ambos grupos fueron subdivididos en dos grupos. A dos de ellos se les administró un segundo tratamiento con amoxicilina en pienso. Los grupos formados fueron: grupo sin tratar, grupo tratado con amoxicilina, grupo tratado con ceftiofur y grupo tratado con ceftiofur y amoxicilina. Durante eltratamiento con ceftiofur se recolectaron muestras fecales antes del tratamiento (día 0), día 2, 7, 14, 21 y 42 después del tratamiento, mientras que durante el tratamiento con amoxicilina, el muestreo se extendió a 73 días después tratamiento. Se seleccionaron E. coli RC y los aislados fueron analizados por electroforesis de campo pulsado (PFGE) y concentraciónmínima inhibitoria (MIC) a 14 antimicrobianos. Además se detectó la presencia de genes de RC y se tipificaron los replicones de los plásmidos. Ambos tratamientos generaron un incremento en la prevalencia de E. coli RC, el cual fue estadísticamente significativo en los grupos tratados, disminuyendo la RC después del tratamiento. Un total de 47 aislados de E. coli RC fueron recolectados durante el estudio; en ellos se encontraron los siguientes genes de resistencia; CTX-M-1 (15), CTX-M-14 (10), CTX-M-9 (4), CTX-M-15 (2) y SHV-12 (5), mayormente asociados a plásmidos conjugativos de los grupos de incompatibilidad IncI1 e IncN. El tratamiento con ceftiofur y amoxicilina fue asociado con la emergencia de E. coli RC durante el curso del tratamiento. Sin embargo, al final del ciclo productivo, antes de que los animales partieran a matadero, no se aislaron cepas de E. coli RC. Los estudios 2 y 3 se llevaron a cabo en ocho granjas de cerdos de engorde convencionales en las que se aplicaban programas de medicina preventiva basados en el uso de antimicrobianos. En ellas se estudió la emergencia de E. coli RC (Estudio 2) y Salmonella RC (Estudio 3) depues del tratamiento con ceftiofur y tulatromicina durante la lactación. Cuatro granjas se trataron con ceftiofur y otras cuatro con tulatromicina. En cada granja, 70 lechones de 7 días de edad fueron divididos en dos grupos; grupo control (n =30) y grupo tratado (n = 40). Se recolectaron muestras fecales a día 0 (antes del tratamiento), día 2,7 y 180 después del tratamiento. Las madres también fueron muestreadas a día 0. Además, se evaluó si los recuentos de E. coli RC con respecto a la población total de E. coli, aumentaban con el uso del tratamiento. En cinco de las ochos granjas, los lechones de 7 días de edad excretaban E. coli RC antes de ser tratados. La prevalencia de animales positivos a E. coli RC fue disminuyendo a medida que aumentaba la edad de los animales. Las tres granjas restantes fueron negativas durante todo elestudio independientemente del tratamiento aplicado. Los resultados mostraron gran variabilidad en la frecuencia de animales positivos en las diferentes granjas. Un aumento transitorio de E. coli RC se observó en los recuentos 48 horas después de haber aplicado el tratamiento con ceftiofur. Sin embargo, otros factores de riesgo, como la presencia de madres positivas y la edad de los animales parecen tener una mayor relevancia en la persistencia de E. coli RC, que el tratamiento aplicado. El Estudio 3, además de evaluar el efecto del tratamiento con ceftiofur o tulatromicina en la emergencia de Salmonella RC investigó la presencia de cepas de Salmonella multirresistentes. Se caracterizaron genotípica y fenotípicamente los aislados obtenidos y se determinó la presencia de diferentes genes de resistencia, así como los plásmidos que portaban dichos genes de RC. Se aislaron 66 cepas de Salmonella en 5 de las ocho granjas. De estas, 49 fueron multirresistentes y 4 de ellas contenían genes blaCTX-M albergados en plásmidos conjugativos de la familia IncI1. Estas cepas RC fueron aisladas en una misma granja antes que los animales fueran tratados con ceftiofur, ydesaparecieron durante el curso del tratamiento. Los genes tet(A) (77%), sul1 (27%), y tet(B) (23%) fueron los más prevalentes, y 10 aislados contenían qnrB que confiere baja susceptibilidad a fluoroquinolonas. En este estudio no se pudo establecer una relación directa entre el uso de ceftiofur y laemergencia de Salmonella RC. Sin embargo la mayoría de los aislados fueron multirresistetes conjuntamente a ampicilina, estreptomicina, sulfamidas y tetraciclinas. Estos antimicrobianos son frecuentemente usados en los programas de medicina preventiva veterinaria. En conjunto, hemos observado un aumento transitorio de cepas de E. coli RC durante el tratamiento con ceftiofur, cosa que no observamos con Salmonella. Sin embargo, cepas RC, tanto de E. coli como de Salmonella, fueron aisladas de animales de siete días antes de someterlos a tratamiento, sugiriendo la circulación de estas cepas en la granja. Hubo animales que partieron a matadero excretando E. coli RC y Salmonella multirresistentes con genes y plásmidos similares a los encontrados en cepas de origen humano. Por lo tanto, nuestros estudios indican que la granja es un reservorio de cepas multirresistentes que pueden llegar al consumidor si entran en la cadena alimentaria. Para minimizar la emergencia de bacterias resistentes, es importante hacer un uso más racional de los antimicrobianos, así como definir estrategias para reducir la circulación de cepas resistentes, tanto dentro de las granjas, como en sistemas integrados de producción.


The main objective of this thesis was to evaluate the association between management practices in relation with antimicrobial consumption in pig farms and the emergence of cephalosporin resistant (CR) E. coli and Salmonella. Moreover, the studies intended to elucidate if the farm was a reservoir of resistance genes harboured in transferable plasmids similar to those found in the community. For this purpose, three studies have been conducted. Study 1 evaluated if the treatments with ceftiofur and amoxicillin are risk factors for the emergence of CR E. coli in a convencional pig farm during the rearing cycle. One-hundred seven-day-old piglets were divided into two groups, control (n=50) and parenterally treated with ceftiofur (n=50). During the fattening period, both groups were subdivided in two. Asecond treatment with amoxicillin was administered in-feed to two of the groups; untreated group, group treated with amoxicillin, group treated withceftiofur and group treated with ceftiofur and amoxicillin. During treatment with ceftiofur, faecal samples were collected before treatment (day 0) and at days 2, 7, 14, 21 and 42 post-treatment, whereas with amoxicillin, the sampling was extended 73 days post-treatment. CR E. coli isolates were genotypically and phenotypically characterised by PFGE, and MIC. The presence of CR genes was assessed and plasmids analysed by replicon typing. Both treatments generated an increase in the prevalence of CR E. coli, which was statistically significant in the treated groups. Resistance diminished after treatment. A total of 47 CR E. coli were recovered during the study period, 15 contained blaCTX-M-1, 10 blaCTX-M-14, four blaCTX-M-9, two blaCTX-M-15 and five blaSHV-12. The treatment with ceftiofur and amoxicillin was associated to the emergence of CR E. coli during the course of the treatment. However, by the finishing time CR E. coli were not recovered from the animals. Study 2 and 3 were conducted in eight conventional pig farms that applied preventive veterinary programmes based on antimicrobial use. In four of these farms, 7-day-old piglets were treated with ceftiofur and in the remaining four with tulathromycin. At each farm, 70 7-day-old piglets weredivided into two groups; a control group (n = 30) and a treated group (n = 40). Faecal samples were taken at day 0 (before medication), 2, 7 and 180 days post-treatment (time of slaughter). Sows were also sampled at day 0. Study 2 also assessed the dynamics of CR E. coli populations during the life cycle. In five of the eight farms, 7-day-old piglets excreted CR E. coli before treatment. The occurrence of CR E. coli positive animals decreased with increasing piglets’ age. The remaining three farms were negative for CR E. coli during the study period. Results demonstrated great variability in the frequency of positive animals between farms, independently of treatment. The treatment with ceftiofur resulted in a transitory increase in the counts of CR E. coli after 48 hours. However, other risk factors, such as the presence of CR E. coli in sows, and the age of the animals had a more relevant role in shedding of CR E. coli than the treatment. Study 3 assessed the effect of the treatments with ceftiofur or tulathromycin in the emergence of CR Salmonella and multiresistant Salmonella. Phenotypic and genotypic characterization of the Salmonella isolates were performed. A total of 66 Salmonella isolates were recovered from five of the eight farms. Of them, 49 were multiresistant and four contained blaCTX-M genes harboured in conjugative plasmids of the IncI1 family. They were recovered from the same farms before treatment with ceftiofur. tet(A) (77%), sul1 (27%), and tet(B) (23%) genes were the most prevalent, and 10 isolates also presented qnrB. A direct relation was not established between the use of ceftiofur and the occurrence of CR Salmonella. However, multidrug resistant was common, especially for ampicillin, streptomycin, sulphonamides and tetracycline.These antimicrobials are the most frequently used in preventive veterinary programmes in Spain. Overall, the treatment with ceftiofur resulted in atransitory increase in both, the proportion of animals shedding CR E. coli and the counts. We did not observed the same increase in prevalence for CR Salmonella. However, CR E. coli and CR Salmonella were detected in 7-day¬old piglets before administration of any treatment, suggesting the presence of resistant strains circulating in the farm. Some of the animals departed to the abattoir excreting CR E. coli and Salmonella with a multiresistant phenotype. CR genes were associated to transferrable plasmids with similar replicons to those found in isolates of human origin. These results indicatedthat the farm is a reservoir of resistant genes and plasmid that can reach the consumer via the food chain. To minimize the emergence of resistant bacteria and protect the consumer health, a more rational use of antimicrobials should be implemented in intensive farming. Intervention strategies should be develop to reduce the re-circulation of resistant isolates within pig farms, as well as within integrated production systems.
