Abstract

INTRODUCCIÓN En el ámbito de la formación de Enfermería, y como consecuencia de la adaptación al Espacio Europeo de Educación Superior, se han producido en estos últimos años numerosos retos para los docentes universitarios. En este contexto, se han generado nuevas necesidades curriculares con actividades inherentes a la investigación, con una mayor diversidad de actividades docentes, seminarios, tutorías, dirección, corrección y evaluación de trabajos de grado y de máster. Al mismo tiempo se requiere una mayor dedicación al alumnado, además de la incorporación de nuevas metodologías educativas. Este proceso de adaptación ha sido complejo y ha comportado numerosos cambios no sólo en la organización del trabajo, sino en otros aspectos psicosociales que pueden repercutir en el bienestar docente. Existen diversos estudios y documentos de organismos oficiales que exploran los factores de riesgo psicosocial y los riesgos psicosociales en el ámbito laboral, incluido el ámbito de la docencia. Sin embargo, en el contexto de los profesores universitarios de Enfermería, existen pocos estudios que identifiquen los factores de riesgo psicosocial. Si bien algunos de estos factores pueden ser comunes a otros colectivos laborales, incluidos los universitarios, y están ampliamente identificados en la literatura, cada profesión y cada contexto laboral tiene unas características propias. OBJETIVOS El objetivo principal de la tesis fue evaluar la relación entre los niveles percibidos de exposición a factores de riesgo psicosocial y el bienestar psicológico en docentes universitarios de Enfermería de Cataluña. Como objetivos específicos, el primero fue identificar los factores de riesgo psicosocial a través de la propia experiencia de estos docentes. A partir de este objetivo se diseñó un instrumento para medir los factores perturbadores del bienestar docente y se analizaron sus propiedades psicométricas. Otro objetivo fue determinar la prevalencia de malestar psicológico y el nivel de salud mental positiva, y analizar su relación con las características sociodemográficas, de salud y estilos de vida. METODOLOGÍA Este estudio se desarrolló en dos fases: en la primera se identificaron los factores de riesgo psicosocial y se construyó un cuestionario para medir la exposición a factores perturbadores del bienestar docente. En la segunda fase se realizó un estudio transversal en una muestra de docentes universitarios de Enfermería para realizar, por una parte, el análisis psicométrico del instrumento diseñado, y por otra, el estudio descriptivo y analítico (determinación de la prevalencia del malestar psicológico, la salud mental positiva y sus relaciones con el resto de variables). El estudio se realizó en las escuelas o facultades de Enfermería de las siguientes universidades de Cataluña: Universitat de Barcelona; Universitat Autónoma de Barcelona; Universitat Rovira i Virgili; Universitat de Girona; Universitat Ramón Llull; Universitat de Vic, Universitat Pompeu i Fabra i Universitat de Lleida. La primera fase se realizó durante el primer semestre de 2013 y la segunda durante el segundo semestre del mismo año. En la primera fase se constituyeron dos grupos de discusión. El primer grupo estuvo formado por 10 docentes y el segundo por 11 docentes. Los criterios de inclusión fueron ser profesor docente universitario de Enfermería, indistintamente de la edad y del sexo, y que en el momento del estudio estuviesen activos. En una primera etapa se les solicitó que identificaran todas aquellas posibles causas perturbadoras del bienestar docente, a partir de la propia experiencia de cada integrante o bien a partir de la experiencia que observaban en compañeros de su entorno laboral. Posteriormente, en una segunda etapa se les solicitó a todos los integrantes de ambos grupos que seleccionaran, de todas las posibles causas que habían identificado, aquellas que consideraban las más relevantes como posibles causas perturbadoras del bienestar docente. Los resultados que se obtuvieron en ambos grupos se agruparon en un listado único de ítems indicadores de posibles causas perturbadoras del bienestar docente. Posteriormente cada ítem fue evaluado en una escala que iba de 1 a 4, donde 1 correspondía a ítem no relevante y 4 a ítem muy relevante. El índice de validez de contenido (IVC) para cada ítem se calculó como el porcentaje de expertos que daban una puntuación de 3 o 4 en cada ítem, incluyendo en el cuestionario aquellos ítems con un IVC igual o superior a 0,80. La población objetivo para la segunda fase fueron todos los profesores de las diferentes universidades de Cataluña (n=1500) que ejercen su labor docente en el ámbito universitario en el Grado en Enfermería. Los criterios de selección de la población de estudio fueron: tener una experiencia mínima de 1 año en la docencia, que en el momento del estudio estuviesen activos y que quisieran participar en el mismo. El tipo de muestreo utilizado fue no probabilístico, ya que los docentes incluidos en el estudio fueron los que voluntariamente quisieron participar. El tamaño de la muestra final de estudio fue de 263 docentes. La recogida de datos se realizó mediante un formulario anónimo diseñado en formato on line para facilitar su cumplimentación y enviado mediante correo electrónico. Se realizó un recordatorio a todos los participantes al mes del inicio del estudio. Se recogieron variables sociodemográficas, de hábitos y estilos de vida y de características laborales y extralaborales. Para medir el malestar general se utilizó el cuestionario de Goldberg en su versión reducida de 12 ítems (GHQ-12), determinando como presencia de malestar una puntuación superior a 2. Para determinar el nivel de salud mental positiva se utilizó el Cuestionario de Salud Mental Positiva (CSMP) de Lluch. Se utilizó el cuestionario diseñado ad hoc en la primera fase para medir el índice o nivel de bienestar docente en los profesores universitarios de Enfermería. Este instrumento se denominó con el acrónimo CuBIDU (Cuestionario de Bienestar Docente Universitario). Está formado por 21 ítems que evalúan tanto la frecuencia como la intensidad de cada uno de ellos. Primero se calculó para cada uno de los ítems obtenidos el producto entre la variable frecuencia con la que se producían las situaciones de malestar y la intensidad experimentada. Para el cálculo del índice total se realizó el sumatorio del total de ítems obtenidos para las 21 situaciones. Para el análisis estadístico se utilizaron las pruebas adecuadas para comparación de variables categóricas y cuantitativas. Para el análisis psicométrico del instrumento diseñado (CuBIDU) se realizó un análisis factorial exploratorio y se determinó la consistencia interna mediante el alfa de Cronbach. Se hizo un análisis discriminante para identificar los ítems del CuBIDU que permitían diferenciar a los docentes según el malestar general (GHQ-12 inferior o igual a 2 y GHQ-12 superior a 2), y cuántos de estos ítems eran necesarios para alcanzar la mejor clasificación posible. Posteriormente se realizó un análisis multivariante mediante regresión logística para determinar la relación entre el CuBIDU y el malestar psicológico ajustado por las otras variables. El estudio fue aprobado por el Comité de Ética e Investigación Clínica de la Fundation Research de Sant Joan de Déu CEIC PIC-65-13. RESULTADOS Los resultados de la primera fase permitieron identificar 21 ítems de factores perturbadores del bienestar docente. Estos 21 ítems fueron los que constituyeron el cuestionario CuBIDU. En el análisis factorial exploratorio del CuBIDU se identificaron tres dimensiones que explicaban el 53% de la varianza: exigencias laborales en el ámbito docente, exigencias curriculares y dificultades en el ámbito organizativo. El coeficiente alfa de Cronbach para el total del cuestionario fue de 0.91 y para cada dimensión se obtuvieron valores superiores a 0.70. Los resultados de la segunda fase se obtuvieron de una muestra de 263 participantes. La edad media fue de 47,8 años (DE 9,9), siendo mayoritariamente mujeres (79,1%) y profesores titulares de la escuela (38,4%). El 93,4% declaró tener relaciones sociales de forma satisfactoria o muy satisfactoria. El consumo habitual de antidepresivos y ansiolíticos fue declarado en el 12,5% y 13,3%, respectivamente. La prevalencia de malestar psicológico en el total de la muestra fue del 27% (21.8 ÷ 32.6). La prevalencia fue superior en los docentes que impartían docencia en el primer y segundo ciclo al mismo tiempo (p=0.02), mientras que fue inferior en aquellos docentes con relaciones sociales y de amistad muy satisfactorias (p=0.01). La prevalencia también fue superior en los docentes que declararon tomar habitualmente antidepresivos (p=0.05), ansiolíticos (p=0.04) y flores de Bach (p=0.03). La puntuación media del Cuestionario de Salud Mental Positiva (CSMP) fue de 114.8 (DE 12.0). Las puntuaciones fueron más elevadas en el factor de resolución de problemas, en el de habilidades de relación interpersonal y en el de autocontrol. El factor con menor puntuación fue el de autonomía. La puntuación media del CSMP fue superior en los profesores titulares de universidad, en los docentes que declararon un grado de satisfacción con las relaciones sociales de forma insatisfactoria y en los que realizaban más de 4 horas de ejercicio físico a la semana (p=0.01, p=0.001 y p=0.04, respectivamente). Se encontró también una relación estadísticamente significativa entre las puntuaciones del GHQ-12 y del CSMP (p=0.0001). La puntuación total del CuBIDU estuvo relacionada con la puntuación del GHQ-12 (p=0.0001), con una puntuación media del CuBIDU superior en los docentes con un Goldberg superior a 2. Las correlaciones entre las puntuaciones del CuBIDU y del CSMP fueron todas negativas y estadísticamente significativas. A medida que aumentaba la puntuación total del nivel de exposición a los factores perturbadores del bienestar docente, la salud mental positiva disminuía. En el análisis discriminante sólo el ítem 2 (Existe un aumento del trabajo on line sin reducción del trabajo presencial), el Ítem 5 (Hay una dificultad para desconectar del trabajo en la vida personal) y el Ítem 20 (Hay una falta de apoyo del equipo directivo del centro o departamento) permitieron clasificar a los docentes en dos grupos según el malestar psicológico. En el análisis multivariante tanto la puntuación total del CuBIDU como el ítem 2 y 5 permanecieron estadísticamente significativos con la puntuación del GHQ-12, independientemente de la edad, el sexo, la salud mental positiva y el consumo de ansiolíticos. CONCLUSIONES Esta investigación ha permitido identificar 21 factores de riesgo psicosociales perturbadores del bienestar docente en una muestra de docentes de Enfermería de Cataluña. A partir de estos factores, según su frecuencia e intensidad, se ha diseñado y validado un cuestionario que permite cuantificar el nivel de exposición a los factores perturbadores del bienestar docente (CuBIDU). La prevalencia del malestar psicológico en la muestra estudiada de docentes de Enfermería ha sido alta comparada con la población general. Esta prevalencia fue superior en docentes que impartían docencia en estudios de máster y grado simultáneamente, en los dos turnos, en docentes con relaciones sociales insatisfactorias y en docentes con consumo habitual de ansiolíticos y flores de Bach. El nivel de salud mental positiva estuvo relacionado con el GHQ-12, con el ejercicio físico y con las relaciones sociales. La puntuación total del CuBIDU estuvo relacionada tanto con la puntuación del GHQ-12 como con la del CSMP. El aumento de trabajo on line sin reducción del trabajo presencial y la dificultad para desconectar del trabajo en la vida personal son factores que influyen en el malestar psicológico de los docentes universitarios de enfermería independientemente de la edad, sexo, consumo de ansiolíticos y nivel de Salud Mental Positiva.


INTRODUCTION Within the setting of nursing education and subsequent to the adaptation to the European Higher Education Area numerous challenges have been produced for university teachers in the last years. In this context, new curricular needs have been generated with activities inherent to investigation, a greater diversity of teaching activities, seminars, tutorship, guidance, correction and evaluation of work related to degree and master classes. At the same time, greater dedication on behalf of the alumni is required in addition to the incorporation of new educational methods. This process of adaptation has been complex and has led to many changes not only in work organisation but also in other psychosocial aspects which may have repercussions on teacher well-being. Several studies and documents of official organisms have evaluated the factors of psychosocial risk as well as psychosocial risk within the laboral setting, including teaching. However, there are few studies on the identification of factors of psychosocial risk within the context of university nursing professors. Although some of these factors may be common to other work collectives, including those in the university which are widely identified in the literature, each professional and laboral setting has its own characteristics. OBJECTIVES The main objective of this thesis was to evaluate the relationship between the levels of exposure to factors of psychosocial risk and the well-being of university nursing teachers in Catalunya. Specific objectives were to first identify the factors of psychosocial risk from the experience of the teachers themselves. With this objective we designed an instrument to measure factors which unsettle teacher well-being and analysed the psychometric properties of this instrument. Another objective was to determine the prevalence of psychological ill-being and the level of positive mental health and analyse their relationship with sociodemographic characteristics, health and life style. METODOLOGY The study was developed in two phases: in the first we identified the factors of psychosocial risk and constructed a questionnaire to measure exposure to factors which affect teacher well-being. In the second phase a transversal study was carried out in a sample of university nursing teachers to determine the psychometric analysis of the instrument designed and to make a descriptive analytical study (determination of the prevalence of teacher ill-being and positive mental health and their relationship with the remaining variables). The study was performed in the nursing schools or faculties of the following Catalonian universities: Universitat de Barcelona, Universitat Autònoma de Barcelona, Universitat Rovira i Virgili, Universitat de Girona, Universitat Ramón Llull, Universitat de Vic, Universitat Pompeu i Fabra and the Universitat de Lleida. The first phase was carried out during the first semestre of 2013 and the second was undertaken in the second semestre of the same year. Two discussion groups were made in the first phase. The first group was made up of 10 teachers and the second included 11. The inclusion criteria were: university nursing teacher, any age or sex and to currently be actively teaching. They were first asked to identify all the possible causes affecting teaching well-being based on the experience of each participant or from observing their co-workers in the laboral setting. Then, the participants in both groups were asked to select what they considered to be the most relevant causes affecting teacher well-being from among all the possible causes identified. The results obtained in both groups were included in a list of items indicating possible causes affecting teacher psychological well-belong. Afterwards, each item was evaluated on a scale from 1 to 4 in which 1 corresponded to a item considered as not relevant and 4 was considered to be very relevant. The content validity index (CVI) for each item was calculated as the percentage of experts who gave a score of 3 or 4 for each item, including only those items in the questionnaire with a CVI greater than or equal to 0.80. The second phase included all the professors from the different universities in Catalunya (n=1500) working within the university setting of Nursing Degree. The selection criteria of the study population included: having at least one year of experience in teaching, currently active and accepting to participate in the study. Non probability sampling was used since the teachers included in the study were those who voluntarily accepted to participate. The final sample size was 263 teachers. Data collection was performed using an anonymous form designed with an online format to facilitate its completion and was sent by email. A reminder was sent to all the participants at one month after the initiation of the study. The variables collected included: sociodemographic, habits and life style and laboral and extralaboral characteristics. The reduced 12-item Goldberg questionnaire (GHQ- 12) was used to measure general ill-being, with ill-being being considered with a score greater than 2. The Lluch Positive Mental Health Questionnaire (PMHQ) was used to determine the level of positive mental health. The ad hoc questionnaire was used to measure the index or level of teacher well-being in university nursing professors. This instrument is denominated CuCIBU (Questionnaire of University Teacher Well-being). This questionnaire is made up of 21 items which evaluate both the frequency and the intensity of each item. First, the product between the variable frequency and that produced in situations of ill-being and the intensity felt was calculated for each item obtained. To calculate the total index, all the items obtained for the 21 items were added together. The statistical analyses were performed using the adequate tests for the comparison of categorical and quantitative variables. For the analysis of the psychometrics of the CuBIDU an exploratory factorial analysis was performed, and the internal consistency was determined using Cronbach’s alpha. A discriminatory analysis was done to identify the items of the CuBIDU which allow the differentiation of the teachers according to ill-being (GHQ-12 less than or equal to 2 and GHQ-12 greater than 2), and which of these items were necessary to achieve the best possible classification. Afterwards, a logistic regression multivariate analysis was made to determine the relationship between the CuBIDU and psychological ill-being adjusted for the other variables. The study was approved by the Ethical Committee of Clinical Investigation of the Fundation Research de Sant Joan de Déu CEIC PIC-65-13. RESULTS The results of the first phase allowed 21 items of factors affecting teacher well-being to be identified. These 21 items were those of the CuBIDU. In the exploratory factorial analysis of the CuBIDO we identified three dimensions which explained 53% of the variance: work demands within the teaching setting, curricular demands and difficulties in the organisational setting. The Cronbach alpha coefficient for the total questionnaire was 0.91, and values greater than 0.70 were obtained for each dimension. The results of the second phase were obtained in a sample of 263 participants. The mean age was 47.8 years (SD 9.9), with a predominance of women (79,1%) and full professores of the school (38,4%). Social relationships were reported as being satisfactory or very satisfactory by 93,4%. Usual consumption of antidepressants and tranquilizers was described by 12.5% and 13.3%, respectively. The prevalence of psychological ill-being in the total sample was 27% (21.8 ÷ 32.6). The prevalence was greater in the teachers working in the first and second cycle at the same time (p=0.02), being lower in those with satisfactory social relationships and friends (p=0.01). The prevalence of ill-being was also higher in teacher who reported normally taking antidepressants (p=0.05), tranquilizers (p=0.04) and Bach flower remedies(p=0.03). The mean score of the PMHQ was 114.8 (SD 12.0). The scores were higher in the factors of problem solving, interpersonal relationship skills and in self-control. The factor with the lowest score was autonomy. The mean PMHQ was greater in full university professors, in the teachers who declared having unsatisfactory social relationships and in those who did more than 4 hours of physical exercise per week (p=0.01, p=0.001 and p=0.04, respectively). We also found a statistically significant relationshp between the GHQ-12 and PMHQ scores (p=0.0001). The total score of the CuBIDO was related to the GHQ-12 score (p=0.0001), with the mean CuBIDU score being greater in teachers with a Goldberg score greater than 2. The correlations between the CuBIDU and PMHQ scores were all negative and statistically significant. The higher the total score of the level of exposure to factors affecting teacher well-being, the lower the positive mental health. In the discriminatory analysis only item 2 (increase in online work with no reduction in teaching work), item 5 (difficulty in disconnecting work from personal life) and item 20 (lack of support from team managers of the centre or department) allowed the teachers to be classified into two groups according to psychological ill-being. In the multivariate analysis both the total score of the CuBIDU and items 2 and 5 remained statistically significant with the GHQ-12 score, independently of the age, sex, positive mental health and consumption of tranquilizers. CONCLUSIONS This study allowed the identification of 21 factors affecting psychosocial risk to teacher well-being in a sample of nursing teachers in Catalunya. According to the frequency and intensity of these factors we have designed and validated a questionnaire which can quantify the level of exposure factors affecting teacher well- being (CuBIDU). The prevalence of psychological ill-being in our study sample of nursing teachers is high compared to the general population. This prevalence was higher in teachers simultaneously giving master and degree classes, doing two shifts, in teachers with unsatisfactory social relationships and in those who usually consume tranquilizers and Bach flower remedies. The level of positive mental health was related to the GHQ-12, physical exercise and social relationships. The total CuBIDU score was related to both the GHQ-12 and the PMHQ score. The increase in online work with no reduction in classes and the difficulty in disconnecting work from the personal life are factors which influence psychological ill-being independently of the age, sex and consumption of tranquailizers and the level of positive mental health.
