Monitorización de la profundidad de la anestesia con potenciales evocados auditivos de latencia media de extracción rápida

Abstract

Introducción: Los potenciales evocados auditivos de latencia media (PEA) obtenidos por la media móvil (MTA) no se usan para monitorizar la anestesia por su prolongado tiempo de actualización. Los PEA obtenidos por el modelo autorregresivo con entrada exógena (ARX) se actualizan en menos tiempo. <br/>Hipótesis: Los PEA-ARX y el índice AAI (AEP-ARX-Index) identifican los cambios que se producen durante la anestesia con propofol o sevoflurano, y podrían ser útiles para monitorizar la anestesia.<br/>Objetivos: Para aceptar la hipótesis como válida, 1. Demostrar que los PEA-ARX se obtienen en menor tiempo pero con igual fiabilidad que los PEA-MTA. 2. Demostrar que el AAI tiene una buena correlación con los PEA-ARX expresada con una probabilidad de predicción (Pk) alta (>0,9) para detectar si el paciente está clínicamente despierto o dormido durante la anestesia con propofol o con sevoflurano.<br/>Trabajo 1: Se analizaron las señales de PEA extraídas simultáneamente con los métodos MTA y ARX y se compararon con la escala Observer Assessment of Alertness and Sedation Scale (OAAS) durante la inducción con propofol (concentración diana: 5mg.ml-1 mediante infusión TCI, manteniendo constante la concentración plasmática a la que se alcanza el OAAS 1, dormido) en 15 pacientes de cirugía cardiaca. <br/>Resultados: (media [DS]) No hubo diferencias significativas en la amplitud ni en la latencia de las ondas obtenidas con ambos métodos. La transición del estado despierto al dormido alargó la latencia de Nb de forma similar para ambos métodos (ARX a 64,2 [4,8] ms; MTA a 63,1 [6,9] ms). El Pk para Na-Pa y para Nb no presentó diferencias significativas con los dos métodos. El tiempo de actualización del modelo ARX fue más corto que el de MTA (6 vs. 28,4 seg.). El tiempo de transición despierto-dormido fue: ARX: 5,4 [24,7] seg.; MTA: 29,8 [27,2] seg. (p<0.05). El Pk entre AAI y OAAS fue 0,93[0,01].<br/>Trabajo 2: Se compararon los PEA-ARX con el índice AAI y la escala OAAS en 24 pacientes intervenidos de cirugía cardiaca anestesiados con propofol o sevoflurano. Propofol: se administra igual que en el trabajo 1. Sevoflurano: fracción inspirada inicial 8% hasta alcanzar el OAAS 1, luego constante a 2,5%. <br/>Resultados: Al pasar de OAAS 5, despierto, a OAAS 1, Nb se alargó a 68,6 (8,1) ms, Na-Pa disminuyó a 0,3 (0,18) mV, y el AAI disminuyó de 74,9 (13,3) a 20,7 (4,7) (P<0,01). Se encontró una correlación significativa entre disminución del AAI, aumento de la latencia de Na, Pa y Nb y disminución de Na-Pa. No se encontraron diferencias significativas entre el grupo anestesiado con propofol o con sevoflurano. Los Pk más altos fueron: AAI (1), Nb (0,94 [0,04] y Na-Pa (0,95 [0,01]). <br/>Discusión: La obtención de los PEA-ARX fue más rápida que los PEA-MTA pero con una configuración similar. El AAI engloba adecuadamente los cambios en los PEA-ARX (Pk AAI-OAAS>0,9). <br/>Conclusiones: Se cumplió la hipótesis propuesta ya que los PEA-ARX y el índice AAI identifican en pocos segundos los cambios que se producen durante la anestesia con propofol o sevoflurano, por lo que pueden ser útiles para monitorizar la profundidad de la anestesia.


Introduction: The Middle Latency Auditory Evoked Potentials (AEP) achieved by moving time average (MTA) is not recommendable for monitoring depth of anaesthesia due to its considerable time delay. The AEP achieved by the autoregressive model with exogenous input (ARX) has a short time delay. <br/>Hypothesis: The AEP-ARX and the AEP-ARX-Index (AAI) correlate to the changes in the depth of anaesthesia in patients receiving propofol or sevoflurane. <br/>Objectives: In order to accept the hypothesis as valid, first it should be demonstrated that the AEP-ARX can be achieved in less time but equally reliable as the AEP-MTA. Second it should be shown that the AAI index has a good correlation to the AEP-ARX in terms of a prediction probability (Pk) (Pk>0.9) to detect if the patient is clinically awake or asleep during the anaesthesia with propofol or sevoflurane.<br/>Study 1: The AEP-MTA and the AEP-ARX were extracted simultaneously and compared to the Observer's Assessment of Alertness and Sedation Scale (OAAS) during the induction with propofol (plasma target concentration: 5 mg/ml during TCI infusion, when OAAS 1 was reached the target was maintained at that particular level) in 15 patients scheduled for cardiac surgery. <br/>Results (Mean (SD)): There was no significant difference in neither latencies nor amplitudes of the AEP-MTA and the AEP-ARX. During the transitions from awake to asleep, the Nb latency was increased similarly with the two methods (ARX 64.2 (4.8) ms; MTA 63.1 (6.9) ms). The Pk values of the two methods did not show significant differences either. The update delay of the ARX method was shorter that that of the MTA (6s vs. 28.4s). The transition time awake to asleep was: ARX 5.4(24.7) s; MTA 29.8 (27.2) s, p<0.05. The Pk between AAI and the OAAS was 0.93(0.01).<br/>Study 2: The AEP-ARX was compared with the AAI and the OAAS scale in 24 patients scheduled for cardiac surgery anaesthetised with propofol or sevoflurane. Propofol: administered as in study 1. Sevoflurane: Fraction inspired 8% until OAAS 1 was reached, thereafter 2.5%.<br/>Results: The change from OAAS 5, awake to OAAS 1, asleep, the Nb was increased to 68.6 (8.1) ms, Na-Pa decreased to 0.3 (0.18) mV, while the AAI decreased from 74.9 (13.3) to 20.7 (4.7) (p<0.01). A clear correlation between the decrease in the AAI, the increase of the latencies of Na, Pa and Nb and the decrease in the amplitude of Na-Pa were found. There was no significant difference between the propofol and sevoflurane groups. The largest Pk values were AAI= 1, Nb= 0.94 (0.04) and Na-Pa= 0.95(0.01). <br/>Discussion: The AEP-ARX was achieved faster than the AEP-MTA but with equal morphology. The AAI encompasses adequately the changes in the AEP-ARX (Pk AAI-OAAS>0.9). <br/>Conclusion: The proposed hypothesis was accepted, as the AEP-ARX and the AAI identify in few seconds the changes in the anaesthetic depth produced by propofol or sevoflurane, hence the two parameters could be useful for monitoring the depth of anaesthesia.
