Medidas terapéuticas de segundo nivel en pacientes con traumatismo craneoencefálico e hipertensión intracraneal refractaria

Abstract

La presente tesis, realizada como compendio de publicaciones, pretende profundizar en el conocimiento de dos de los tratamientos médicos considerados como de segundo nivel en los pacientes con TCE: el coma barbitúrico y la hipotermia moderada.<br/>1) Pentobarbital versus thiopental in the treatment of refractory intracranial hypertension in patients with traumatic brain injury: a randomized controlled trial. <br/>El objetivo principal es estudiar la efectividad del pentobarbital y el tiopental para controlar la PIC en pacientes con un TCE grave e hipertensión intracraneal (HIC) refractaria a las medidas de primer nivel. También se evaluó la presencia de efectos adversos.<br/>Resultados: Se incluyeron 44 pacientes. No hubo diferencias estadísticamente significativas en las características basales salvo en la tomografía computerizada. En 11 pacientes del grupo de tiopental (50%) y en 18 del grupo de pentobarbital (82%) la HIC no se pudo controlar (p=0.03). En el modelo de regresión logística, el tiopental fue más eficaz que el pentobarbital en el control de la PIC (OR=5.1; IC 95% 1.2-21.9; p=0.0027). No hubo diferencias entre ambos grupos en la incidencia de hipotensión arterial o infecciones.<br/>Conclusiones: El tiopental parece ser más eficaz que el pentobarbital para controlar la HIC refractaria a medidas de primer nivel. Estos resultados deben ser interpretados con cautela debido a que las características de los TC no eran similares en ambos grupos. La incidencia de efectos adversos fue similar.<br/>2) Effect of barbiturate coma on adrenal response in patients with traumatic brain injury. <br/>El objetivo principal es evaluar la función adrenal en los pacientes con un TCE que requieran ser tratados mediante coma barbitúrico.<br/>Material y métodos: estudio prospectivo realizado en 40 pacientes. El grupo A fueron 17 pacientes tratados mediante coma barbitúrico. El grupo B incluyó a 23 pacientes con un TCE con HIC controlada mediante medidas de primer nivel.<br/>Resultados: Una vez inducido el coma barbitúrico, los pacientes del grupo A presentaron una mayor incidencia de insuficiencia adrenal comparando con el grupo B (53% vs 22%; p=0.03). <br/>Los pacientes tratados mediante coma barbitúrico y que presentaron una insuficiencia adrenal requirieron dosis más altas de noradrenalina que el grupo de pacientes en coma barbitúrico pero sin insuficiencia adrenal: 1.07±1.04 microgramos/Kg/minuto vs 0.31±0.32 microgramos/Kg/minuto; p=0.03).<br/>Conclusiones: Los pacientes con un TCE y que son tratados mediante coma barbitúrico tienen un mayor riesgo de presentar una insuficiencia adrenal. Este subgrupo de pacientes requirieron una mayor cantidad de soporte vasoactivo.<br/>3) Intravascular cooling for rapid induction of moderate hypothermia in severely head-injured patients: results of a multicenter study (INTRACOOL).<br/>Los objetivos de este estudio son comprobar la factibilidad, seguridad y eficacia de los métodos intravasculares para inducir HT moderada en pacientes con TCE grave e HIC refractaria.<br/>Resultados: 24 pacientes con un TCE grave e HIC refractaria fueron tratados con HT moderada. Se consiguió alcanzar la temperatura diana de 32.5ºC en una mediana de tiempo de 3 horas. La PIC antes del inicio del inicio de la HT fue de 23.8 mmHg y se redujo a 16.8 mmHg tras alcanzar la temperatura diana. <br/>Las causas de muerte en los 9 pacientes que fallecieron fueron: en cuatro casos se produjo un reascenso de la PIC durante el recalentamiento, un paciente presentó una hiperpotasemia producida por una sobrecarga accidental de potasio, dos casos por shock séptico, un paciente por una parada cardiaca de etiología desconocida y el último por un tromboembolismo pulmonar. <br/>Conclusiones: Los métodos intravasculares, combinados con la infusión de suero fisiológico frío a 4ºC durante la inducción de HT moderada, son factibles y eficaces para reducir la PIC en pacientes con un TCE grave e HIC refractaria a medidas de primer nivel.


The next work, made by published articles, tries to get a better understanding about two of the medical treatment that are commonly used in TBI patients, as second tier treatments: the barbiturate coma and moderate hypothermia.<br/>1) Pentobarbital versus thiopental in the treatment of refractory intracranial hypertension in patients with traumatic brain injury: a randomized controlled trial.<br/>The main objective of the present study was to assess the effectiveness of pentobarbital and thiopental in terms of controlling refractory intracranial hypertension in patients with severe TBI, and to evaluate the adverse effects of treatment.<br/>Results: A total of 44 patients were included. There were no statistically significant differences in baseline characteristics, except for admission computed cranial tomography characteristics. Uncontrollable intracranial pressure occurred in 11 patients (50%) in the thiopental group and in 18 patients (82%) in the pentobarbital group (p=0.03). Under logistic regression analysis, thiopental was more effective than pentobarbital in terms of controlling intracranial pressure (OR=5.1; IC 95% 1.2-21.9; p=0.0027). There were no differences between the two groups with respect to the incidence of arterial hypotension or infection.<br/>Conclusions: Thiopental appeared to be more effective than pentobarbital in controlling intracranial hypertension refractory to first-tier measures. These findings should be interpreted with caution because of the imbalance in cranial tomography characteristics. The incidence of adverse effects was similar in both groups.<br/>2) Effect of barbiturate coma on adrenal response in patients with traumatic brain injury. <br/>To evaluate the adrenal function in TBI patients undergoing barbiturate coma, as treatment of their refractory intracranial hypertension. <br/>Materials and methods: We prospectively studied 40 patients. Group A (17 patients) were treated with barbiturate coma and group B (23 patients) presented intracranial hypertension controlled with first tier measures, and acted as control.<br/>Results: Once barbiturate coma was induced, patients in group A presented higher incidence of adrenal insufficiency compared with the control group B (53% vs 22%; p=0.003). Patients treated with barbiturate coma, who developed adrenal impairment, required higher doses of norepinephrine than patients treated with barbiturates without adrenal impaiment (1.07±1.04 micrograms/Kg/minute vs 0.31±0.32 micrograms/Kg/minute; p=0.03).<br/>Conclusions: Patients with TBI treated with barbiturate coma are at higher risk of developing adrenal insufficiency. This subset of patients presented higher requirements of vasoactive support.<br/>3) Intravascular cooling for rapid induction of moderate hypothermia in severely head-injured patients: results of a multicenter study (INTRACOOL).<br/>The objectives are to evaluate the feasibility, safety and effectiveness of a new method of intravascular management of inducing moderate hypothermia (MHT).<br/>Results: 24 patients with severe TBI and refractory high ICP were treated with MHT. <br/>MHT was attained in a median time of 3 hours. Preenrollment median ICP was 23.8 mmHg and was reduced to 16.8 mmHg upon reaching target temperature. Of the nine patients who died, in four the cause was rebound ICP during rewarming, one death was attributed to accidental potassium overload, two to septic shock, one to cardiac arrest on unknown origin and the ninth to a pulmonary embolism.<br/>Conclusions: Intravascular methods to induce MHT combined with precooling with cold saline at 4ºC appear to be feasible and effective in reducing ICP in patients with high ICP refractory to firs-line therapeutics measures.
